If there has been an accident, injury, or near‑miss at one of our facilities, please use this form to report the incident as soon as possible. Accurate reporting helps us keep the facility safe for all users and ensures that any hazards are dealt with promptly.
When completing the form, please include the following vital details:
- Date and time of the incident
- Location (MUGA, Village Hall, Park, Cemetery etc)
- Name(s) of the person/people involved
- Description of what happened
- Type of injury or issue (if applicable)
- Names and contact details of witnesses
- Any immediate actions taken (first aid, calling emergency services, isolating equipment, etc.)
